With over 40+ years of experience in the healthcare industry, he has owned and operated several rehabilitation companies throughout the east coast. Bill has distinguished himself as an outstanding and innovative problem-solver and leader who is well respected within the Long Term Care industry and the greater healthcare community.
Kristin Diamond has been with Care Ventures Inc., since early 2006 when she started with the company as a Human Resources Assistant. She quickly grew in her role becoming a Human Resources Generalist in 2007 advanced to the Director of Human Resources in 2010.
Throughout that time, she has gained experience in both Skilled and Outpatient Rehabilitation Services, and all support departments that are managed in the Corporate Office in Cumberland, MD. Kristin was promoted to the Director of Human Resources and Communication in late 2011 where she oversees the human resources, recruiting, IT and Care Ventures staff.
She is an active member of SHRM and the Healthcare Workforce Advisory Council.
Chelsea is an occupational therapist and has 19+ years of experience in long-term care rehabilitation and management. She has spearheaded and managed therapy programs throughout the Mid-Atlantic region. Recognized as an accessible, forward-thinking and solution-oriented leader, Chelsea oversees the entirety of Flagship Rehabilitation’s day-to-day operations, as well as corporate compliance. She is actively involved in the redesign and implementation of cutting-edge programs and products to meet the future needs of the healthcare industry.
In 2005, Dustin Freas joined Flagship Rehabilitation. Currently the Chief Development Officer, Dustin is a licensed Nursing Home Administrator (NHA) in Maryland and Pennsylvania. He is also a licensed business consultant, evaluating and solving organizational inefficiency. Dustin meets regularly with state and federal policymakers to promote measures that increase access, lower costs and improve the quality of healthcare. He is a strong advocate for a consumer-centered health care system, value-based payment design, and the expansion and utilization of health information technology.
Nick is the Chief Financial Officer for Flagship Rehabilitation. He holds a Master’s in Business Administration from West Virginia University, majoring in Accounting. Nick is also an acting notary. His previous experience includes public accounting for various healthcare and not-for-profit entities.
Andréa Beall serves as the Director of Marketing for Flagship Rehabilitation. She has a Brand Management Certification from Cornell University and is a Certified Dementia Practitioner from NCCDP. Andréa has over 10 years of healthcare marketing and communications experience with Life Plan Communities, Outpatient Clinics and Home Health. Her marketing and branding background includes experience in developing and executing successful advertising campaigns, digital marketing and social media outreach, web development, CRM programs, corporate identity and branding, internal communications and media relations strategies. Previously, Andréa held the position as Director of Outpatient Services responsible for daily oversight, supervising and managing, completing annual reviews and educational training. Andréa actively participates in corporate partnerships with state and national affiliations to increase awareness and support advocacy for aging services.
Our Director of PDPM Operations for Flagship Rehabilitation, Ashley K. Haltenhof, has focused on understanding and educating our Flagship Team and Partners in Medicare’s Patient Driven Payment Model. As a Certified Dementia Practitioner through the National Council of Certified Dementia Practitioners and International Council of Certified Dementia Practitioners, Ashley has specialized in dementia and other neurodegenerative diseases in Long Term Care Communities. Ashley graduated from George Washington University with a Bachelor of Arts Degree. She then went on to successfully complete her Master of Science Degree in Speech Language Pathology from West Virginia University. In addition, she has obtained her RAC-CT certification to further enhance her knowledge and provide education with healthcare providers, nurses, and administration in the Long Term Care industry to improve efficiency, clinical outcomes, resident care, and best practices. Throughout Ashley’s career, she has worked as a Speech Language Pathologist, Clinical Specialist, Director of Rehabilitation, Regional Director, and Clinical Program Consultant.
Ashley is continually developing programs focusing on providing education and improving operations in healthcare. She has extensive experience in the areas of operational analysis of Long Term Care providers, Medicare and Medicaid reimbursement, regulatory compliance, state and federal survey assistance, the Minimum Data Set 3.0 (MDS 3.0), Mock Survey and the Resident Assessment Instrument (RAI) process. Ashley has a strong expertise in program development, as well as program implementation where she trains, mentors, and educates clinical teams at all levels. Ashley’s clinical program specialties include dementia, restorative nursing, dysphagia, and neurological disorders.
Autumn Wade has been with Flagship Rehabilitation since 2001. A proven clinical operations leader in various healthcare settings, Autumn started her career as a occupational therapist and was subsequently promoted to various operational roles. Currently, as Director of Outpatient/Long Term Care Services, Autumn is responsible for the clinical operations of Outpatient and Medicare B services for Flagship Rehabilitation.
Autumn attended Penn State University, Pennsylvania, where she obtained a Bachelor of Science degree in Occupational Therapy. She has completed numerous clinical and operational continuing education classes and continues to practice as an occupational therapist. Autumn is instrumental in the development of educational programs for Flagship Rehabilitation and provides expert insight through her certifications; Certified Lymphedema Therapist, Certified Stroke Rehabilitation Specialist and Certified Dementia Practitioner.
Autumn’s leadership promotes engagement and strengthens partnerships through evidence-based practice to achieve outcomes at every level of the continuum.
Regina Slater has over 12 years of medical speech pathology experience with a specialty in caseload development and progressive neurological disorders. Her comprehensive clinical experience spans across medical settings including outpatient clinics, acute care, tracheostomy/ventilator specialty hospitals, TBI continuum of care community re-integration, Aging In Place Assisted Living and Independent Living campuses, home health and skilled nursing facilities. With extensive experience as a Regional director and Compliance officer in multiple settings Regina is an expert at developing, monitoring and maintaining strong rehabilitative departments from both a clinical outcomes and operational process and procedure perspective. She has a passion for educating physicians and clinicians and has presented at ASHA on the importance of clinically specialties, provided community education and overseen the clinical and professional mentorship of Clinical Fellows every year.
Bethany Alt, COTA/L, CSRS, is a Compliance Officer. She oversees compliance programs ensuring adherence to internal policies as well as State and Federal regulatory and legal requirements. She leads a team of compliance professionals who establish procedures to monitor policy development and conduct audits and investigations. Bethany is an Occupational Therapist Assistant, is highly skilled at implementing compliance best practices, including comprehensive risk assessments and annual audit and compliance plans focusing on preventing, identifying, and reconciling compliance matters consistent with Flagship’s strategy and vision. She is a Certified Lymphedema Therapist and a Certified Stroke clinician.
Jean Clark, RN, is a Compliance Officer for Flagship Rehabilitation. She leads compliance, billing and coding, denials management, and credentialing. Jean is responsible for performing in-depth analysis of clinical billing data to identify documentation, coding, and denial prevention. She promotes revenue cycle operational efficiency, data integrity, and billing and regulatory guidelines of compliance. Jean coordinates complex denials with community IDT members to identify the root cause. She works closely with clinical areas to effectively document services performed and understand the relationship of documentation, medical necessity, coding, and charging for all services provided.
