Short Term
Flagship Rehabilitation provides a team approach to care. We work closely with your IDT to ensure each resident’s Plan of Care is achieved.
Flagship Rehabilitation will implement, enhance or customize the following programs along with any special program requests you may have. We will work closely with your Nursing and Administration staff to ensure successful implementation of person-centered programs. Our management team will conduct ongoing auditing to monitor program outcomes. All our programs are developed by therapists and nurses with physician input. Auditing of current program and implementation structure will be provided along with in-services to your community. Any additional programs selected will be at an additional cost.

Flagship will implement our strong CMI Management systems to ensure the needs of your residents are addressed.
Memory Care
Flagship Rehabilitation provides therapy services to your residents with Dementia. Our goal is to improve cognition, provide memory support along with improving functional ability such as; balance, muscle strength, and mobility and provide pain management.
Flagship has Certified Dementia Practitioners (CDP) ® and has a qualified CDP trainer who presents seminars to Certify Dementia Practitioners. The Management team and Site Managers have received this training. Our Clinical Therapy Directors have trained their staff in how to communicate with a person with Dementia, how to handle personality and behavior changes that occur through using creativity, using patience, demonstrating flexibility, and leading with compassion.
Assisted Living

Independent Living
Flagship Rehabilitation combines cutting edge technology and proven therapy programs to improve outcomes, promote Aging-In-Place, and to assist residents achieve their highest level of function. Comprehensive Physical Therapy, Occupational Therapy and Speech Therapy services are delivered in the same way in these environments.

Flagship Rehabilitation offers two options for Outpatient Therapy Services:
Outpatient Therapy Services – Essentials.
Flagship Rehabilitation will assist your community in obtaining your Medicare Provider number to offer outpatient therapy services to your surrounding community.
Outpatient Therapy Services – Extend Includes all the feature of Essentials, plus:
Flagship offers other complementary services such as Restorative Management Services, Rehab Management Services, etc. to uniquely tailor our partnerships with the communities we serve.
Voyager Wellness transitions members from therapy services to wellness services without interruption of care. Once goals are met in rehab, the resident will transition to wellness to maintain their functional level. Our team begins discharge planning two weeks before therapy discharge. Voyager Wellness provides individual and group programming for AL/PC, IL, and employees.
Our Starboard Home Health program provides full service on and off campus home health therapy management and staffing services. We will partner with your home health agency or with community-based home health providers that serve your campus. Residents will experience a personalized transition of care maintaining the same clinician through multiple Post-Acute Care settings.
